Submitted information is stored to manage your inquiry, waitlist entry, privacy request, or Buddy Pass verification request. With the authorization collected on patient forms, contact details, location, and selected interests may be shared with a participating practice to arrange follow-up.
Independent practices
A participating practice is responsible for its own care decisions and privacy practices. Its notices and consent process apply when you establish a relationship or receive treatment.
Storage and access
Requests are stored in the website’s hosted database. Access, retention periods, deletion procedures, and any additional processors must be documented by the operator before public launch. No advertising pixels or optional analytics have been added to this version.
Your choices
You can ask to access, correct, or delete submitted information, or withdraw your authorization for future sharing, through Privacy Requests. Identity verification may be required. Withdrawal does not undo sharing that already occurred.
