Requests and appointments
Submitting a form records an inquiry. It does not confirm an appointment, reserve a treatment, guarantee a nearby practice, or secure a Buddy Pass. A practice must separately confirm its availability and terms.

Provide accurate information that you are authorized to share. Do not submit medical records, misuse the forms, impersonate another person, or interfere with the website.
Prices and memberships
Any displayed starting price is informational. Your practice confirms the included treatment components, total price, payment terms, membership benefits, and cancellation rules before you commit.
Buddy Pass program
Buddy Pass participation is subject to availability, clinical eligibility, and the separate Buddy Pass Terms. Joining a waitlist is free and does not guarantee a pass.
